About the role

Deliver high-quality community-based care for children and young people with complex needs, support smooth transitions, provide consultancy and training to education providers, and ensure evidence-based, client-focused services and coordinated care plans.

Key duties of the role

  • Provide effective and efficient medical consultation services to community-based children and young people
  • Ensure the development of a comprehensive Health Care Plan in consultation with relevant health professionals for children and young people in educational settings
  • Ensure the transition to home, community and adult services be coordinated in a seamless manner.
  • Monitor and respond to existing and evolving health needs.
  • Provide a range of service delivery models that are strongly grounded in the best available evidence

Skills and Experience

  • Bachelor of Medicine; Bachelor of Surgery (MBBS) or equivalent.
  • Appropriate Specialist Qualifications and registrable with the Australian Health Practitioner Regulation Agency
  • Clinical experience in child, adolescent or community health medical management
  • Experience in, and commitment to, an interdisciplinary approach to services
  • High level skills in negotiation, mediation and conflict resolution.
