Step into a rewarding dual-site role across Keith and Bordertown where you’ll coordinate high-quality programs, social support, and essential home-based services.

Consideration will be given if job sharing of this position is requested, with 0.4FTE at Keith Day Centre only and 0.4FTE at Bordertown Home Support Co Ordinator only.

Key Responsibilities:

  • Plan, deliver, and evaluate group and individual activity programs that support independence and wellbeing.
  • Provide personal care, assistance with daily living tasks, and support for carers.
  • Coordinate and run the social and respite program at Keith Day Centre.
  • Support home-based services for clients through the Bordertown Home Support Program.
  • Collaborate with the multidisciplinary team to ensure coordinated, high-quality care.
  • Participate in health promotion and early detection activities as required.

We serve a population of over 67,000 people in the Limestone Coast, as well as residents from Western Victoria. We employ over 1400 people, with 1.2% identifying as Aboriginal and Torres Strait Islander and we are also supported by a team of approximately 600 hardworking volunteers.

This is a part time (0.8FTE) vacancy, working 60 hours per fortnight and salary will be adjusted according to actual hours worked.
