About the Role

Eyre and Far North Local Health Network (EFNLHN) is seeking a dedicated Home Care Coordinator to join Country Health Connect – Community & Allied Health, based in Streaky Bay.

In this role, you will provide comprehensive care management for Commonwealth funded aged care programs, including Support at Home and the Commonwealth Home Support Program. You will work closely with older people, carers, service providers and multidisciplinary teams to assess needs, coordinate services and support clients to remain living safely and independently at home.

Key Duties

  • Conduct holistic assessments and develop, implement and review client-centred care plans
  • Coordinate referrals and services across health, community and government providers
  • Advocate for clients while promoting independence, choice and self-advocacy
  • Support service improvement through data collection, evaluation and quality initiatives
