WHAT ARE WE OFFERING?

Working within the Community Nursing Team - based in Victor Harbor, you will provide high quality nursing care in community-based settings such as client homes and in clinics within the Southern Fleurieu region. As part of the multidisciplinary community team, you will comprehensively and holistically assess clients, plan and implement goal centred care and evaluate client progress. The role includes management of wounds, catheters and paediatric assessment. You will monitor client care plans to ensure appropriate care outcomes are achieved on a daily basis and oversee the provision of nursing care within the community nursing team.

You will provide care in a variety of community settings, including client homes and clinics, supporting individuals to remain well, independent and supported close to home.

HOW WILL YOU CONTRIBUTE?

  • Provide safe, high-quality, person-centred nursing care to clients in community settings under the direction of a Registered Nurse.
  • Undertake nursing assessments and contribute to planning, implementation and evaluation of goal-centred care plans.
  • Deliver clinical care within scope of practice, including wound care, catheter management and paediatric assessments.
  • Monitor client care plans daily and report changes in condition to support optimal care outcomes.
  • Work collaboratively within the multidisciplinary community nursing team to ensure coordinated and holistic care.
