Are you an Enrolled Nurse passionate about delivering person-centred care and supporting older adults to maintain independence at home?

Join the Yorke and Northern Local Health Network (YNLHN) Healthy Ageing Team and make a meaningful difference by providing high-quality nursing care and support to consumers and their carers in the community. This role works autonomously within a community setting, supporting consumers and support workers with a strong emphasis on advocacy and person-centred decision-making. You will work closely with Registered Nurse Clinical Leads, receiving professional direction and support while contributing to positive health outcomes that enable people to live safely and independently at home.

Key Responsibilities:

  • Support the Registered Nurse/Midwife in providing safe, person-centred care.
  • Deliver community-based nursing services that promote client independence.
  • Complete assessments for new clients and annual reviews within the Healthy Ageing Team.
  • Assist carers in supporting clients’ ongoing health and wellbeing.
  • Work under supervision while maintaining accountability for nursing practice.

What We’re Looking For:

We’re seeking a compassionate, reliable Enrolled Nurse with experience providing community or aged care nursing. You’ll be confident supporting clients in their homes, completing assessments, and working collaboratively with the Healthy Ageing Team. Strong communication skills, professionalism, and a commitment to promoting independence and quality care are essential. You must be enrolled or eligible for enrolment with the Nursing and Midwifery Board of Australia, holding (or eligible to hold) a current practising certificate.

This is a part time vacancy, working 32 hours per fortnight and salary will be adjusted according to actual hours worked.
