About the role

The Youth Health Worker engages with vulnerable young people to access and navigate primary health care through case management, assessment and psychosocial support. The role works collaboratively with families and agencies to improve outcomes, build self-management capacity and drive service improvement.

Key duties of the role

  • Facilitate vulnerable young people’s access to primary health care
  • Use knowledge, evidence and experience to achieve agreed patient care outcomes
  • Identify improvements in the provision of services to support adolescent & young adult development
  • Collaborate with clients in the development and delivery of their case management/care plans
  • Identify, plan and coordinate the delivery of projects/programs

Skills and Experience

  • Appropriate Degree or equivalent qualification which gives eligibility for full membership of the Australian Association of Social Workers.
  • Advanced interpersonal and communication skills
  • Planning, facilitation, evaluation and delivery of group-based projects for young people
  • Experience providing assessment, case planning and health care services to young people with complex needs
  • Knowledge of adolescent psycho-social development, mental health and youth health issues
