Play a vital role in supporting people with disability to achieve their goals, exercise choice and control, and access the supports they need to live well in their community. This rewarding role offers the opportunity to work collaboratively within a multidisciplinary team while developing your expertise in coordination, systems improvement and person-centred service delivery.

About the Role:

As the NDIS Coordinator, you will work as part of a multidisciplinary team to assist in the implementation, coordination and evaluation of NDIS programs delivered through Country Health Connect, a registered NDIS provider. You will liaise closely with participants, families, carers and external agencies to support the identification and achievement of individual NDIS plan goals.

Under the general direction of the Disability and Brokered Services (DABS) Program Manager, you will contribute to the smooth operation of NDIS programs and services by creating, implementing and evaluating support and administrative systems and processes. You will ensure services are consumer-directed, responsive to participant choice, and delivered within plan budgets, organisational capacity and agreed risk parameters.

What We’re Looking For:

  • Experience in working in a community setting.
  • Demonstrated experience in collation and analysis of data Experience in working within a multidisciplinary team.
  • Experience in Word, Excel, Email.

  • Immunisation Risk for this position is - Category A
