About the role

As Social Worker, you will support vulnerable young people to access and navigate Youth Health services through assessment, case management, and targeted program. The Social Worker delivers high-quality, empathetic care and strengthens protective factors for young people.

Key duties of the role

  • Facilitate the access of vulnerable young people to relevant primary health care solutions.
  • Use knowledge, evidence and experience to achieve agreed patient care outcomes.
  • Coordinate service options for young people to build their capacity to manage their own health care support.
  • Deliver case management services that engage young people in their health care management.
  • Work collaboratively with clients in the development and delivery of their individual case management/care plans.

Skills and Experience

  • Appropriate Degree or equivalent qualification which gives eligibility for full membership of the Australian Association of Social Workers
  • Knowledge of Metropolitan Youth Health population groups, psychosocial support and group work models and practices.
  • Knowledge of adolescent psycho-social development, mental health and youth health issues.
  • Experience in assessment, case planning and health care services within a case management approach to young people.
  • Experience in planning, facilitation, and evaluation of group-based projects for young people.

About MYH: 

  • MYH works on prevention, early intervention, and youth specialist health response measures regarding adverse childhood events (ACE) and their impact on young people’s health, including speciality programs for young parents.
  • This position is located at a MYH clinical health service location site, where multiple support programs run, including various grant funded Young Parents support Programs.
  • MYH programs focus on building and strengthening safe and positive relationships for all young people accessing our health services but with special focus on young parents and their children.
  • Building these protective factors improve children and young people’s resilience and reduce the impacts of ACE’s on their future health and disrupting intergenerational cycles.
