SA Health is seeking a dedicated Clinical Nurse to join our Home Rehabilitation team at Flinders Medical Centre within the Southern Adelaide Local Health Network. This full-time, ongoing position offers the opportunity to deliver exceptional patient care in the comfort of patients' homes across Adelaide's southern metropolitan region.

About the role: 

The Clinical Nurse will work within the Interdisciplinary Ambulatory Home Rehabilitation team, providing rehabilitation nursing to patients in the community following recent discharge from hospital.  The role reports to the Home Rehabilitation Manager and Nurse Unit Manager.  This role is predominately clinical in nature, providing advanced clinical assessment, treatments and case management to patients admitted to the service.  The incumbent requires a high level of autonomy, excellent communication skills, advanced clinical nursing skills, and the ability to work within an interdisciplinary team in order to support patient care.

Duties for the role will include:

  • Provide proficient, person centred, clinical nursing care and individual case management to patients/clients admitted to the service
  • Keeping up to date with, implementing and monitoring evidence-based practice and quality management initiatives consistent with organisational policies.
  • Required to, within pre-determined guidelines, and in a inter-disciplinary primary health care setting, assess clients, select and implement different therapeutic interventions, and/or support programs and evaluate progress
  • Dealing appropriately with patients and their families where there can be multiple complexities, diverse cultural backgrounds and consumer expectations.
  • Accepting responsibility for the maintenance of own knowledge and professional competence and contemporary practices.
  • Working Autonomously in patients' homes which can be diverse environments.
  • Undertake specific activity and/or portfolio responsibility

About you:

  • Registered Nurse/Midwife with at least 3 years, full time equivalent, post registration experience
  • Experience in the management of complex wounds including vac dressings, PICC care/dressings and venepuncture
  • Must hold current South Australian driver's license and willingness to drive a government plated vehicle
  • Effective communication, problem solving, conflict resolution and negotiation skills
  • Ability to prioritise workload and meet set timelines, whilst working under minimal supervision
  • Appropriate clinical skills to be able to work autonomously in the community and apply independent decision-making whist in the patient’s home
  • Proven experience in basic computing skills, including email and word processing.
  • A Nurse who Accepts accountability for their own practice standards, activities delegated to others and the guidance and development of less experienced staff.
