About the Role

Join a dedicated multidisciplinary team supporting older people with complex needs to stay safe, well and independent at home. You will manage short term caseloads, build tailored care plans and deliver holistic, person centred support across community settings.

• Deliver short term case management for complex older clients
• Assess client needs and plan tailored care and make appropriate onward referrals
• Build links with families, carers and health teams
• Advocate for client choice, safety and autonomy
• Support discharge and avoid hospital admission

About You

You are a compassionate clinician who enjoys working with older people and delivering holistic, person centred care in the community.

• Social Work degree or AHPRA registered OT
• Experience with complex older clients and case management
• Strong communication and teamwork skills
• Ability to work independently and prioritise tasks
• Knowledge of aged care, and community care
