About the Role:

Northern Adelaide Local Health Network (NALHN) is eager to recruit an experienced Physiotherapist to work in our dynamic, growing and supportive team, to help us deliver exceptional care to our community.

As a Senior Physiotherapist – Acute Care Navigator (SWIFT), you will lead the development, implementation, coordination, and evaluation of high‑quality physiotherapy services to assist in progression of care for acute inpatients. Working from Thursday to Sunday, you will collaborate with other members of the Supportive Weekend Interprofessional Flow Team (SWIFT) and multidisciplinary partners across medical, nursing, and allied health services. You will play a pivotal role in optimising care progression, supporting discharge planning, and improving overall patient flow.

As a highly skilled clinician, you will deliver advanced physiotherapy assessment, treatment, and care‑navigator services, ensuring timely identification of patients suitable for proactive care planning and barrier resolution. You will also provide clinical leadership within the acute hospital environment, offering guidance, support, and supervision to junior staff while contributing to ongoing service improvement.

To be eligible for this position, you must hold an appropriate Degree or equivalent qualification which entitles registration with the Physiotherapy Board of Australia.

  • proven ability to build productive relationships with patients, families, community agencies, and multidisciplinary teams
  • leadership capability, including staff supervision, workload prioritisation, adaptability, and the ability to work both independently and collaboratively
  • experience in service development, policy implementation, and evaluation within an acute care setting
  • commitment to continuous professional learning aligned with NALHN objectives
