Join Our Specialist Drug and Alcohol Team - RN2 Clinical Liaison Role

Join SA Health as a part-time RN2 Clinical Liaison Nurse in our Drug and Alcohol Services, where you will coordinate seamless care transitions between hospital and community settings for patients with substance use disorders. About the role: 

Reporting to the Regional Manager, this role is responsible for coordinating comprehensive care pathways between acute hospital settings and community drug and alcohol services, ensuring seamless transitions and optimal outcomes for patients with substance use disorders.

Duties for the role will include:

  • Providing proficient clinical nursing care to patients referred to the hospital Consultation Liaison Service with high severity alcohol and drug related problems and concurrent comorbid medical and mental health issues,
  • Monitoring patient/client care plans to ensure appropriate care outcomes are achieved on a daily basis,
  • Oversee the provision of nursing care within a team/unit,
  • Assists and supports the Nurse Unit Manager or equivalent in management, clinical, and education activities,
  • Plan and coordinate services including those from other disciplines,
  • Act to resolve local and/or immediate nursing care or service delivery problems,
  • Support change management processes

About you:

  • Registered or eligible for registration as a Nurse by the Nursing and Midwifery Board of Australia and who holds, or who is eligible to hold, a current practicing certificate,
  • Effective communication, problem solving, conflict resolution and negotiation skills,
  • Ability to work effectively within a multidisciplinary team,
  • Ability to prioritise workload and meet set timelines, whilst working under minimal supervision,
  • Demonstrated competence in the relevant area of nursing practice in accordance with the relevant standards of practice,
  • Experience in the leadership and direction of student nurses, enrolled nurses and less experienced registered nurses
