Lead and coordinate high-quality stroke care across hospital and community settings, improving patient outcomes through clinical leadership, education and service development.

About the Role

As the Stroke Nurse Coordinator, you will provide expert clinical leadership and consultancy in stroke management across the network.  Working closely with multidisciplinary teams, you'll drive best-practice care, strengthen service coordination, and support improved outcomes for stroke patients in regional communities.  

You will play a key role in developing stroke services, delivering education, monitoring performance, and leading quality improvement initiatives aligned with national stroke targets.

What We're Looking For

  • Registered (or eligible) with the Nursing and Midwifery Board of Australia, with a current practising certificate
  • Minimum 3 years post-registration experience
  • Demonstrated clinical competence (stroke experience desirable) and commitment to evidence-based, person and family centred care
  • Strong leadership, communication, and problem-solving skills, with the ability to influence practice and lead change
  • Experience working collaboratively across multidisciplinary teams and building partnerships with internal and external stakeholders
  • Commitment to culturally respectful care, including experience working with Aboriginal consumers and communities
  • Proven experience in quality improvement, audits, project implementation, and supporting staff or students, with proficiency in Microsoft Office
