  • Ongoing role working 34.96 hours per fortnight over a 5-day roster 

 

WHAT ARE WE OFFERING?

The Nurse Consultant Chronic Disease role offers the opportunity to provide expert clinical nursing care and leadership to ensure quality and safety in chronic disease management, including cardiac and respiratory care, across a variety of clinical settings. This role enables you to work with a high level of autonomy, supporting improved patient/client outcomes through evidence-based practice and advanced clinical expertise.

In this role, you will contribute to clinical leadership, coordination of programs and service improvement initiatives, while supporting multidisciplinary teams, enhancing models of care, and assisting clients to develop self-management strategies to improve their health and wellbeing.

Please Note: role is regional with a location to be negotiated.

HOW WILL YOU CONTRIBUTE?

  • Provide expert clinical nursing care, assessment, case management and therapeutic interventions for clients with chronic disease.
  • Support integrated care through clinical advice, education, discharge planning and hospital avoidance strategies.
  • Lead and contribute to quality improvement, risk management and clinical governance processes.
  • Utilise information systems to inform decision making, monitor outcomes and support safe service delivery.
  • Provide leadership, mentorship and support to nursing and multidisciplinary teams.
  • Contribute to education, research, and the development of clinical protocols and best practice standards.

WHAT ELSE DO YOU NEED TO KNOW/WHAT IS ESSENTIAL FOR SUCCESS?

  • You will need to undergo employment screening, what you require is also on the front page of the Role Description.
  • You must be registered or eligible for registration as a Nurse with the Nursing and Midwifery Board of Australia and hold, or be eligible to hold, a current practicing certificate.
  • You must hold a contemporary Professional Practice Portfolio containing evidence of a relevant post graduate qualification or be currently working towards a relevant post graduate qualification.
