About the Role

Join as a Transitional Care Consultant and support safe discharge and hospital avoidance across mental health services. Identify barriers, build care pathways and coordinate packages that improve flow and consumer outcomes.

• Identify barriers and plan early discharge pathways
• Coordinate hospital avoidance and discharge packages
• Work with inpatient teams and community providers
• Educate staff on alternatives to hospital care
• Maintain activity data and support service improve

About You

You’re an experienced mental health nurse ready to support complex discharge and hospital avoidance. You bring strong leadership, clinical insight and the ability to coordinate care across services.

• Registered Nurse with current AHPRA registration
• Mental health qualification or willingness to obtain
• Experience in complex care coordination and consultancy
• Lead change and work across multidisciplinary teams
• Analyse data and support quality improvement
