  • Salary: ASO3 $71,589 - $ 76,033 p.a. (pro rata) + Super + Salary Packaging
  • Fulltime Permanent and Temporary to June 2027, contracts available.

About the Role

As a Placement and Home Brokerage Officer, you coordinate safe transitions from hospital to community care. You manage brokerage pathways, build strong partnerships and help older people access the right services for timely, supported discharge.

  • Coordinate aged care placements and discharge pathways
  • Prepare and submit admission paperwork to providers
  • Liaise with patients, families and carers on care options
  • Work with multidisciplinary teams to support patient flow
  • Build relationships with providers, LHNs and agencies
  • Maintain accurate records and update care plans

About You

You are organised, adaptable and people focused, with strong communication skills and the ability to manage competing priorities while supporting safe, timely patient transitions.

  • Strong communication with diverse audiences
  • Provide timely, high level admin support
  • Work to deadlines and adapt to change
  • Use Microsoft Office to produce quality work
  • Experience in healthcare or similar environment
