About the Role

Join the CO-ACT team and deliver high quality care for older people transitioning into community or residential aged care supports. You will manage care, build capability in others and maintain safe, person centred practice within a supportive team.

What you’ll do:

  • Deliver safe, person centred patient care
  • Assess, plan and evaluate care needs
  • Supervise ENs, AINs and students
  • Delegate tasks and support team growth
  • Maintain clear, accurate clinical records
  • Build strong team and partner relationships
  • Communicate with patients, families and services
  • Support safe, smooth care transitions

About You

You’re a confident and compassionate Registered Nurse who brings initiative, teamwork and a genuine passion for older persons care.

  • Registered Nurse with NMBA registration or eligibility
  • Hold or eligible for current practising certificate
  • Communicate with clarity, empathy and confidence
  • Work collaboratively across multidisciplinary teams
  • Prioritise care, solve problems and adapt to change
