About the Role

Deliver high-quality occupational therapy within CO-ACT at Hampstead, supporting older people with complex needs. Work with autonomy to assess, plan and guide care, build strong partnerships and help shape safe, effective transitions and outcomes.

  • Assess complex patient needs and plan care
  • Deliver therapy to support independence and maintenance of function
  • Guide safe discharge and transition pathways
  • Collaborate with multidisciplinary teams
  • Liaise with community and residential services
  • Provide advice and education to staff and carers
  • Contribute to quality improvement and research

About You

You bring strong clinical capability, sound judgement and a genuine commitment to person-centred care for older people across complex settings.

  • Registered with OTBA and hold recognised OT degree
  • Apply clinical reasoning across complex caseloads
  • Communicate clearly with patients, carers and teams
  • Manage workload, prioritise tasks and meet deadlines
  • Maintain accurate records and use clinical systems
