  • Salary  $91,785.00 - $106,330.00 p.a. (Pro Rata) AHP2 | Full Time – Contract available until October 2026.

Geriatric Evaluation and Management (GEM) Units services provide specialised high-quality care to older South Australians for problems that are unique to old age. Our GEM services are staffed by a multidisciplinary team as a Social Worker in our team, you will be a involved in optimising care and outcomes for our patients by providing a comprehensive range of social work services to patients requiring specialist geriatric evaluation, assessment and care.  Click here to find out more! 

You will work with other members of the team to effectively plan and provide therapeutic interventions based on patient goals. This could involve consultation and liaison with the team, other professional disciplines and non-government organisations. 

About the role: 

Reporting to the Allied Health Team Leader - GEM and the Director of Social Worker - RAP, some of your duties as a Social Worker may involve: 

  • Independently assessing clients' abilities, problems and needs using a variety of formal and informal assessments as needed. 
  • Planning and implementing evidence-based intervention programs. 
  • Documenting your assessment findings and interventions.
  • Prescribing and arranging for the provision of appropriate services.
  • Monitoring and improving the standards of services provided to clients.

Please note: The successful candidate will work across multiple teams/sites including Noarlunga Hospital, Repat Health Precinct and Flinders Medical Centre, as required. 

About you:

  • A Degree or equivalent qualification in Social Work, entitling membership of the Australia Association of Social Workers. 
  • Experience in the social work management of older people with complex health and care needs. 
  • Experience as a social worker in a healthcare setting. 
  • Demonstrated competence in the application of a wide range of assessment and therapeutic interventions to clients with complex needs. Demonstrated ability to provide social work clinical services as a member of an interdisciplinary team. 
  • Extensive experience in geriatric clinical care is an advantage.
