Overview:

  • The Clinical Project Manager will use their clinical knowledge and experience to in relation to the redesign and commissioning of service pathways required to contribute to the development of the state-wide projects, inclusive of the reform initiatives including leading the liaison function between SA Health and the Transition to Home Program.
  • The Clinical Project Manager works collaboratively with the multidisciplinary team and both clinical and management staff to contribute to major system change.
  • The Clinical Project Manager will contribute to advice, leadership, management, and clinical knowledge for the reform initiatives including supporting the delivery of health reform.
  • The Clinical Project Manager will contribute as an integral team member of the Integrated Care Team contributing to the Integrated Care Team business plan and leadership team.

Criminal History Screening Requirement

  • A ‘National Police Certificate’ NPC (also known as a ‘Nationally Coordinated Criminal History Check’ NCCHC) for employment involving no contact with vulnerable groups is required for this position (general employment).
  • A DHS ‘Working With Children Check’ (WWCC) is required for this position.

Immunisation Risk for this position is Category B (indirect contact with blood or body substances).

SA Health Services are required to implement the Addressing vaccine preventable disease: Occupational assessment, screening and vaccination policy in the workplace.

Special Conditions

  • Registered Nurse/Midwife with at least 3 years post registration experience.
  • Experience at a senior management level in consulting with senior clinicians, Health Services and Non-Government Organisations.
  • Experience in the development and implementation design and project management of complex service redesign projects including prioritising and managing complex tasks, determining priorities, leading teams and meeting timelines.
  • Experience in the preparation, implementation and evaluation of policies, strategic plans and strategies dealing with complex issues.
  • Experience in patient transitions between acute care and the community sector including NDIS and aged care.
  • An appropriate tertiary qualification in a health or business-related discipline or project management.
  • Understanding of supported discharge strategies especially in supporting transition to Commonwealth funded aged care programs and the National Disability Insurance Scheme.
