    • AHP1: $73,710.00 p.a. - $89,981 pa (pro rata basis)  
    • AHP2: $94,997 p.a - $110,052 p.a. (pro rata basis) 
  • Permanent, Part Time – 30 hours per week 

   Appointment to this position will be at either AHP1 or AHP2 classification, commensurate with the applicant’s level of professional experience, demonstrated competence, and capacity to perform with increasing independence, clinical judgement, and complexity of practice as outlined in the role requirements. 

 

WHAT ARE WE OFFERING?  

The Physiotherapist provides client-centred, goal-oriented and outcome-focused care to older people across the Barossa Hills Fleurieu Local Health Network, working within a multidisciplinary Community Reablement team. The role supports individuals accessing Commonwealth-funded aged care programs across clinic, residential, and home settings, with a focus on maintaining and improving independence, delaying functional decline, and reducing reliance on ongoing services. Using a reablement and wellness approach, the Physiotherapist delivers targeted, time-limited interventions to support issues such as reduced mobility, falls risk, cognitive impairment, frailty, and post-acute recovery needs. 

The role involves collaboration across the client journey to support restorative goal setting and improved wellbeing, independence, and autonomy. Working with increasing professional autonomy, the Physiotherapist may also provide clinical supervision and support to less experienced staff, allied health assistants, and students. In addition, the role contributes to a culture of continuous learning, supports complex case management, and promotes team-based care to ensure consistent, high-quality physiotherapy outcomes within the community aged care setting.

HOW WILL YOU CONTRIBUTE?  

You will……….   

  • Deliver client-centred, goal-oriented physiotherapy services to older people across clinic, home, and residential settings within Commonwealth-funded aged care programs, using a wellness and reablement approach to optimise independence and functional capacity. 
  • Undertake comprehensive assessment, intervention planning, implementation, and evaluation for a diverse caseload, including conditions such as frailty, falls risk, cognitive impairment, chronic disease, and post-acute rehabilitation needs. 
  • Work collaboratively within a multidisciplinary team to support coordinated care, discharge planning, referrals, and integrated service delivery, while contributing to improved client outcomes across the care continuum. 
  • Provide clinical leadership, supervision, and support to allied health assistants, students, and less experienced staff, while maintaining professional practice standards and contributing to a culture of learning and capability building. 
  • Contribute to service development, quality improvement, data collection, reporting, and compliance with program and accreditation requirements, ensuring efficient, evidence-based, and sustainable service delivery.
