About the role

Prevent non-medical admissions and deliver psychosocial and crisis support to children, young people, and families at WCH. Provide care in PED and beyond, and drive service improvements within multidisciplinary and interagency teams.

Key duties of the role

  • Collaborate and engage with external providers to prevent non-medical admission where possible
  • Work in collaboration with key stakeholders in the PED service to develop, implement and support service delivery for consumers presenting to the PED on a regular basis
  • Use high-level professional knowledge and skill to manage referrals, provide social work assessments, make interpretations of assessment findings, and formulate and implement social work intervention plans in collaboration with patients, carers and relevant other clinicians.
  • Provide advice to the Social Work Manager on professional service development, practice and redesign, in response to clinical demand and client needs.
  • Liaise closely with families, staff of other disciplines and relevant community agencies to provide an integrated approach to client management.

Skills and Experience

  • Appropriate Degree or equivalent qualification which gives eligibility for full membership of the Australian Association of Social Workers.
  • Demonstrated ability to independently apply a high level of professional judgement relevant to the clinical specialty.
  • Advanced ability to implement patient centred and evidence-based practice
  • Substantial and recent experience in an area of social work clinical specialty to enable input at a senior clinician level.
  • Experience in supporting consumers and their families within paediatric clinical environment
  • Willingness to work hours including evenings, public holidays and weekends
