About the Role

Step into a pivotal role shaping high-quality care for older people in the Transition Care Program. You will lead clinical practice, build strong multidisciplinary partnerships, and create innovative approaches that support safe, seamless care and positive outcomes.

• Lead clinical care across the Transition Care Program
• Guide nurses and allied health staff
• Build strong team and partner links
• Drive projects that lift care quality and outcomes
• Ensure safe, consistent care and policy alignment
• Support skill growth through mentoring and feedback

About You

You’re a confident clinical leader who brings energy, sound judgement and a passion for improving care for older people.

• Registered Nurse with current AHPRA registration
• Strong clinical expertise in aged or transition care
• Lead teams with clear, calm communication
• Improve models of care with a person-centred focus
• Know NSQHS standards and Aged Care Act 2024
