  • Employment: Temporary Part Time Contract - (up to 2 July 2027) | Monday - Friday (0800 to 1630)

About the Role

As a Patient Liaison Enrolled Nurse, you'll play an important role in supporting the safe and timely discharge of patients with more complex needs,working closely with patients, families, medical and nursing teams, community services and external providers.

You will:

  • Coordinate complex discharges, ensuring assessment, planning, documentation and follow-up are completed.
  • Connect patients, families and care teams with appropriate community services and providers to support continuity of care.
  • Promote early, appropriate discharge while contributing to education, service coordination and ongoing quality improvement.

Essential Criteria

  • Enrolled or eligible for enrolment with Nursing & Midwifery Board of Australia and hold, or be eligible for, a current practicing certificate.
  • Experience providing nursing care in an acute healthcare setting.
  • Effective verbal and written communication skills.
  • Ability to work collaboratively within a multidisciplinary team.
  • Ability to prioritise workload and recognise and report changes in condition.
  • Commitment to delivering person-centred care.
