This position is multi-classed as either AHP2 or RN/M2C and the appointment will be determined by qualifications, experience and capability.
WHAT ARE WE OFFERING?
We are offering a rewarding 2-month contract opportunity as a Case Manager within the Transitional Care Packages (TCP) program. In this role, you will work across a variety of health service settings, delivering high-quality nursing and/or midwifery care while operating with increasing autonomy as a proficient practitioner. You will take accountability for your own clinical practice, contribute to the supervision and development of less experienced staff, and support the Nurse/Midwife Unit Manager in leading the team. This role provides the chance to strengthen your leadership capability, contribute to continuity and consistency of care, and play an active part in implementing practice improvements within a supportive and collaborative environment.

WHO ARE WE LOOKING FOR?
Are you…..
- A Registered Nurse or Nurse/Midwife (or eligible) with AHPRA registration and a current practising certificate, bringing at least 3 years post-registration experience or Clinical Nurse/Midwife classification.
- A collaborative and values-driven professional who consistently demonstrates trust, respect, integrity, kindness, and accountability in their practice.
- A strong communicator with well-developed interpersonal skills, able to engage effectively with diverse communities, including Aboriginal consumers, in a culturally appropriate manner.
- A highly organised and adaptable, with the ability to prioritise workload, work independently, and apply innovative thinking in a dynamic healthcare environment.

HOW WILL YOU CONTRIBUTE?
You will……….
- Deliver high-quality, person-centred nursing/midwifery care and case management within the Transition Care Program, ensuring care plans are effectively monitored and aligned with Commonwealth TCP Guidelines.
- Support service delivery by assisting the Nurse/Midwife Unit Manager with clinical operations, coordinating multidisciplinary care, and proactively resolving care or service issues.
- Contribute to education and workforce development by mentoring students and junior staff, fostering a positive learning environment, and supporting ongoing professional development.
- Engage in quality improvement and evidence-based practice through participation in clinical audits, research activities, and the integration of best-practice knowledge into patient care.
- Provide leadership within the team by promoting continuity of care, supporting patient flow, and contributing to a safe, collaborative workplace culture focused on continuous improvement and innovation.
