About the role

CALHN Integrated Care provides services to individuals who do not require the resources of a general hospital but are beyond the scope of the traditional primary health care services. The focus is on preventing unnecessary hospital admissions and facilitating early discharge pathways from the acute sector.

In this role, the pharmacist will have opportunities to work across the Integrated Care service including SA Urgent Care Hubs, Geriatrics in the Home and Hospital in the Home. The pharmacist role is highly valued by the team and you will play a key role in delivering optimal medication management services, combining your clinical expertise with a passion for excellence in patient care to ensure best possible outcomes for patients. As part of CALHN Pharmacy to you will have access to the full range of expertise available in our dispensing, electronic medication management, manufacturing, investigational drugs, clinical, and medication governance teams. This role will be on a seven-day roster to support the range of services provided by Integrated Care.

In this role, you will exercise a high level of professional judgement and autonomy to support safe, effective and cost conscious medication management practices. Working closely with pharmacists, technicians, assistants and healthcare professionals, you will contribute to service improvement initiatives, optimise quality use of medicines and help ensure pharmacy services are delivered in line with professional standards, hospital policies and best practice care outcomes.

About You

You are an experienced and motivated Pharmacist who brings strong clinical expertise, leadership capability and a commitment to delivering safe, high quality pharmacy services within a complex healthcare environment.

• Registered Pharmacist with Pharmacy Board of Australia
• Extensive hospital pharmacy and leadership experience
• Lead teams and improve pharmacy service delivery
• Analyse data and solve complex clinical challenges
