About the Role:

You will provide person-centred nursing care and case management to support the health and wellbeing of clients within the community. You will assess individual needs, coordinate services with multidisciplinary teams and contribute to quality improvement activities that promote safe and effective care delivery. You will also support health education, participate in ongoing professional development, and provide guidance to less experienced staff and students.

About You: 

  • Registered or eligible for registration with the Nursing and Midwifery Board of Australia with a current practising certificate.
  • Strong communication, problem-solving and teamwork skills within a healthcare environment.
  • Ability to prioritise workload and provide safe, consumer and family-centred care.
  • Demonstrated competence in community nursing practice in line with professional standards.
