Eyre and Far North Local Health Network is seeking a compassionate and motivated Enrolled Nurse to join the Country Health Connect team as a Care Partner based in Ceduna.

In this rewarding community-based role, you will support people receiving Support at Home and Commonwealth Home Support Program (CHSP) services, helping them maintain independence, wellbeing and quality of life within their own homes and communities.

About the Role

Working under the direction and supervision of a Registered Nurse, you will contribute to the delivery of person-centred care through clinical support, care coordination, monitoring and advocacy.

Key responsibilities include:

  • Delivering safe and effective nursing care within scope of practice
  • Supporting participants to remain independent at home
  • Monitoring wellbeing and escalating clinical concerns appropriately
  • Assisting with care planning and service coordination
  • Maintaining accurate clinical documentation
  • Supporting participants, carers and families through education and guidance

About You

To be successful in this role, you will demonstrate:

  • Enrolled Nurse registration with the Nursing and Midwifery Board of Australia
  • Minimum 3 years post-registration experience
  • Strong communication and teamwork skills
  • Commitment to person-centred care and community wellbeing
  • Ability to work independently and within multidisciplinary teams
  • Current driver’s licence and willingness to travel
