About the Role

Join the CALHN Cancer Program as a CAN Connect Nurse Consultant and play a key role in supporting cancer patients through every stage of their care journey. You will coordinate the CAN Connect service, providing expert clinical assessment, advice and supportive care via phone, telehealth and in-person consultations to help patients access the right care at the right time.

Working with a high degree of autonomy, you will collaborate closely with multidisciplinary teams to coordinate care pathways, support symptom management and improve patient outcomes. You will also provide clinical leadership, contribute to service development and quality improvement initiatives, and help ensure cancer patients receive responsive, accessible and person-centred care.

About You

You are an experienced and compassionate nurse who is passionate about delivering exceptional cancer care. You bring strong clinical expertise, leadership skills and the ability to build trusted relationships with patients, families and healthcare teams.

• Registered Nurse with NMBA registration or eligibility

• Lead cancer care coordination and patient support

• Build strong multidisciplinary partnerships

• Support service improvement and clinical excellence

• Mentor staff and promote positive patient outcomes
