About the Role

Support palliative care patients and families where your work brings comfort, dignity and guidance through complex psychosocial needs. You’ll deliver counselling, coordinate support and contribute to holistic care within a compassionate multidisciplinary team.

Key Responsibilities

• Deliver counselling and bereavement support to patients and families
• Assess psychosocial needs and plan tailored care responses
• Contribute to interdisciplinary care planning and reviews
• Provide specialist advice and consultancy to staff and services
• Liaise with teams and community palliative care partners

About You

You are a compassionate Social Worker who brings clinical skill, empathy and resilience to supporting people through complex palliative care needs.

• Eligible for AASW membership with relevant social work degree
• Deliver psychosocial assessment and counselling in health care
• Apply family counselling and grief support in sensitive settings
• Work effectively within multidisciplinary teams and services
• Manage complex cases with sound judgement and cultural awareness
