About the Role:

As a Social Worker (AHP2) within the Hospital in the Home (HITH) Mental Health Service, you will provide high-quality assessment, intervention and care coordination services that support consumers to achieve optimal mental health outcomes in a community-based setting. Working with a diverse caseload, you will deliver evidence-based, recovery-oriented social work services, including comprehensive psychosocial assessment, treatment planning, therapeutic interventions and discharge planning in partnership with consumers, families, carers and other health professionals.

Operating with a degree of professional autonomy, you will apply your clinical expertise to manage complex and non-routine situations, while contributing to safe, effective and person-centred care. You will work closely with a multidisciplinary team of medical, nursing and allied health professionals, as well as community agencies and support services, to ensure coordinated care and positive consumer outcomes.

The role offers opportunities to contribute to service development, quality improvement, research and student supervision, while continually developing your own clinical and professional capabilities. You will play an important part in fostering a culture of collaboration, continuous learning and excellence within the Northern Mental Health Service.

You are a qualified and compassionate Social Worker with a strong commitment to recovery-oriented mental health care. You bring excellent assessment, communication and interpersonal skills, thrive in a multidisciplinary environment, and are confident managing complex situations with professionalism and empathy.

Organised and adaptable, you can effectively prioritise a varied caseload while building positive relationships with consumers, families, carers and community partners to support meaningful health and wellbeing outcomes.

Our Local Health Network Sites and Services:

NALHN provides a range of hospital (inpatient), outpatient, community and home-based services across multiple sites in Adelaide’s north and north-eastern suburbs, including Aboriginal health, mental health, and GP services.

Lyell McEwin Hospital – the major hospital in northern Adelaide for emergency care, complex and multi-day surgery, medicine, obstetric, paediatrics and outpatient services.
