About the Role:

You will provide advanced clinical care, leadership and care coordination to support people to remain safely in their communities and reduce avoidable hospital presentations. Working closely with multidisciplinary teams, acute services and community providers, you will facilitate timely discharges, support care planning and contribute to initiatives that improve patient flow, access to services and health outcomes across the region.

942780 RN/M2A Role Description

About You: 

  • Registered or eligible for registration as a Nurse/Midwife with the Nursing and Midwifery Board of Australia and hold, or be eligible to hold, a current practising certificate.
  • Registered Nurse/Midwife with at least 3 years post-registration experience or currently classified as a Clinical Nurse/Midwife.
  • Demonstrated competence in the relevant area of nursing practice.
  • Strong communication and teamwork skills, with experience supporting less experienced nursing staff.
