About the role: 

Reporting to the FMC Acute Social Work Manager, this role is responsible for the delivery of high-quality services to patients on acute wards.

Duties for the role will include:

  • Developing, negotiating and delivering services that are relevant to the consumers psychosocial needs
  • Ensure active participation of patients and their families/ carers in their assessment and  early discharge planning
  • Developing collaborative relationships with the multi-disciplinary team
  • Initiating, planning and participating in education programs designated for students, medical, nursing and other allied health staff promoting social work values and theories.
  • Contributing to the development of an integrated team approach and culture which is highly responsive to the needs of our consumers.
  • Maintaining accurate clinical documentation in line with SA Health standards and professional obligations

About you:

  • Appropriate Degree or equivalent qualification which gives eligibility for full membership of the Australian Association of Social Workers
  • Communicate effectively both verbally and in writing
  • Use interpersonal skills to build therapeutic relationships with patients and families 
  • To work effectively in a multi –disciplinary team using negotiation skills and collaboration to ensure safe outcomes for patients and their families
  • Use social work skills to undertake psycho-social assessments and intervene with patients   and their families to effect change
  • Advocate on behalf of patients and their families
  • Work under pressure and meet deadlines and prioritise work appropriately
