About the Role

Use your skills to advocate for older people facing complex geriatric needs who are living in the community. As a Social Worker within Integrated Care Geriatric Services, you will provide comprehensive assessments, support and short-term case management to help clients navigate complex health needs, social challenges and services.

• Provide comprehensive assessments, support and short-term case management
• Advocate for older people with complex health needs and social challenges
• Assist families and carers with complex care planning and accessing services
• Deliver counselling and therapeutic interventions
• Build partnerships across multidisciplinary teams

About You

You are a compassionate Social Worker with sound clinical judgement who is passionate about supporting older people through holistic, person-centred care.

• Social work degree and eligible for AASW full membership
• Manage complex geriatric caseloads with confidence
• Deliver holistic assessments and case management
• Build strong partnerships across health and community services
• Advocate for older people and their wellbeing
