About the Role:

Join Country Health Connect as a NDIS Coordinator and make a meaningful difference by helping people with disability achieve their goals and live more independently. Working within a supportive multidisciplinary team, you'll coordinate person-centred services, build strong relationships with participants and their support networks, and deliver high-quality, tailored support that promotes choice, independence and positive outcomes.

What We're Looking For:

Essential Criteria 

  • Strong communication and relationship-building skills with participants, families, carers and stakeholders.
  • Ability to work independently and collaboratively, using sound judgement and initiative to achieve outcomes.
  • Strong organisational, analytical and problem-solving skills, with the ability to manage competing priorities.
  • Experience working in community and multidisciplinary settings, with proficiency in Microsoft Office applications.
  • Knowledge of confidentiality principles and person-centred, consumer-directed care.

Desired Criteria 

  • Certificate III or IV in Aged Care, Community Services or Disability Services.
  • Knowledge of the National Disability Insurance Scheme (NDIS) and financial reporting requirements.
  • Positive, adaptable attitude with a willingness to learn and embrace change.
