Overview:

The Clinical Program Manager will provide advice, leadership, management and clinical knowledge for state-wide strategic projects and reform initiatives related to the Department for Health and Wellbeing (DHW) Hospital in the Home and community based post-acute services, with a focus on health (including mental health), aged care, disability, and priority populations. They will be required to:

  • Coordinate projects relevant to the implementation of Hospital in the Home and community-based post-acute services with relevant community-based stakeholders.
  • Provide advice, leadership, management, and knowledge of the Hospital in the Home and community based post-acute service implementation and related initiatives to DHW and SA Government agencies as required.
  • Significantly contribute to the Hospital Community Integration Team and Integrated Care Systems Directorate.

Criminal History Screening Requirement

  • A ‘National Police Certificate’ NPC (also known as a ‘Nationally Coordinated Criminal History Check’ NCCHC) for employment involving no contact with vulnerable groups is required for this position (general employment).
  • A DHS ‘Working With Children Check’ (WWCC) is required for this position.

Immunisation Risk for this position is Category C (minimal patient contact).

SA Health Services are required to implement the Addressing vaccine preventable disease: Occupational assessment, screening and vaccination policy in the workplace.

Special Condition

  • Appropriate degree or equivalent qualification which gives eligibility for full membership of the Australian Association of Social Workers, or,
  • Appropriate degree or equivalent qualification and general registration with the Psychology Board of Australia,
  • Appropriate Degree or equivalent qualification which entitles registration as an Occupational Therapist with the Occupational Therapy Board of Australia or,
  • Appropriate Degree or equivalent qualification which entitles registration as a Physiotherapist with the Physiotherapy Board of Australia.
