This is an opportunity for an experienced Social Worker to bring clinical depth and collaborative energy to a multidisciplinary team making a real difference in northern Adelaide.
Your Role:
Working part time on a fixed-term contract through to 30 June 2027, you will provide high-quality, evidence-based social work services as part of the Allied Health Complex Care Team, located at the Lyell McEwin Health Hospital with part of this role providing dedicated social work services at Calvary Hospital, working closely with medical, nursing and allied health professionals to support consumers with complex health and psychosocial needs across acute and community settings.
Your key responsibilities will include:
- Delivering comprehensive psychosocial assessments, crisis intervention and therapeutic support for patients with complex needs.
- Leading and coordinating complex discharge planning to facilitate safe, timely and person-centred transitions from hospital to community-based care.
- Liaising with patients, families, carers, community providers and government agencies to address barriers to discharge, including housing, social supports, guardianship and service access.
- Collaborating with medical, nursing and allied health colleagues to develop and implement integrated care and discharge plans.
This role offers an excellent opportunity to work within a collaborative multidisciplinary environment across Lyell McEwin Health Service and Calvary Hospital, supporting patients with complex needs to achieve safe and sustainable outcomes both during admission and following discharge.
What You Bring
- Tertiary qualification in Social Work and eligibility for full membership with the Australian Association of Social Workers (AASW)
- Substantial post-qualifying experience in an acute hospital or health setting
- Demonstrated ability to manage high-complexity caseloads with sound clinical judgement
- Proven experience in discharge planning within a multidisciplinary team environment
- Strong understanding of the social determinants of health relevant to northern Adelaide's diverse community
- Current SA Working with Children Check (or eligibility to obtain)
To be eligible for this position, you must have an Appropriate degree or equivalent qualification which gives eligibility for full membership of the Australian Association of Social Workers
