Social Worker – Rehabilitation | Flinders Medical Centre | SALHN | Contract to December 2026

Join Flinders Medical Centre's Rehabilitation team as a Social Worker and help southern Adelaide patients reclaim independence - contract through to December 2026.

About the role: 

The Social Worker participates as a member of the inter-disciplinary team ensuring optimal care and outcomes by providing a comprehensive range of social work services to patients needing rehabilitation.

Duties for the role will include:

  • Responding to referrals and contributing to the formulation of plans to assist clients and their carers in rehabilitation, discharge from hospital and reintegration into the community
  • Social assessments and planning and in conjunction with the multidisciplinary team deliver individual and group interventions
  • Advocating for and on behalf of individuals and groups including ensuring the exchange of relevant information about client and carer needs as well as optimising clients’ and carers’ use and access to health, government and community resources
  • Planning and implementing intervention programs
  • Communicating with patients’ family, carers and other service providers
  • Assessing patients’ abilities, problems and goals using a variety of formal and informal assessments

About you: 

  • A degree and or post graduate qualification which gives eligibility for full membership of the Australian Association of Social Workers
  • Ability to work effectively as a member of a multidisciplinary team.
  • A tolerant and non-judgemental approach to all staff and clients.
  • Ability to apply a wide range of social work methods
  • Experience in working with other staff to plan and implement service improvements, quality activities or research
  • Experience in working as a member of an interdisciplinary team.
  • High level interpersonal, negotiating, counselling and problem-solving skills with the ability to communicate with patients, other professional staff and community agencies both orally and in writing.
