Geriatric Evaluation and Management (GEM) Units services provide specialised high-quality care to older South Australians for problems that are unique to old age. Our GEM services are staffed by a multidisciplinary team as a Social Worker in our team, you will be involved in optimising care and outcomes for our patients by providing a comprehensive range of social work services to patients requiring specialist geriatric evaluation, assessment and care. 

About the role: 

Reporting to the Allied Health Team Leader - GEM, GEM and the Director of Social Worker - RAP, some of your duties as a Social Worker may involve: 

  • independently assessing clients’ abilities, problems and needs using a variety of formal and informal assessments as needed
  • planning and implementing evidence-based intervention programs
  • participating in multidisciplinary clinical and planning activities to enhance the overall service provided to clients
  • documenting assessment findings and interventions in accordance with service standards
  • collaborating with other team members to initiate, develop, implement, and evaluate therapy/activity groups
  • supervising students allocated to the ambulatory programmes and undertaking teaching activities
  • providing a consultative service to other team members, disciplines and agencies.

About you:

  • A degree or equivalent in Social Work, entitling membership of the Australian Association of Social Workers. 
  • Experience in the social work management of older people with complex health and care needs
  • Demonstrated competence in the application of a wide range of assessment and therapeutic interventions to clients with complex needs
  • Demonstrated ability to provide social work clinical services as a member of an interdisciplinary team.
  • Ability to critically analyse or evaluate issues related to clinical care in social work in a manner which can be applied to quality activities, service evaluation or research.
  • Ability to work under pressure, prioritise workloads and meet deadlines.
  • High level interpersonal and negotiating skills and the ability to communicate with clients and staff at various levels, both orally and in writing
  • Primary location Noarlunga, however, travel to other sites (Flinders & Repat) may be required.
