About the role

As a Senior Pharmacist, you’ll play a key role in establishing a new position in the SALHN Geriatric Outreach Service (GOS), supporting services reaching into Residential Aged Care Facilities, in both metro and country locations. GOS aims to deliver specialist geriatric services to support older people who have dementia/progressive cognitive decline and are experiencing behaviours due to their dementia to:

  • Transition from hospital into residential aged care.
  • Prevent avoidable admissions into hospital from residential aged care.

You will be working with nursing, medical and allied health staff as part of a multidisciplinary team. Experience with telehealth is desirable to support the rural in reach services. In addition, you will bring your expertise and professional judgement to provide independent, autonomous pharmacy services, medicines information and therapeutic advice to patients, medical and nursing staff, and other health professionals. Working within hospital and divisional policies, procedures and professional standards, you’ll be a trusted and valued member of the healthcare team.

You’ll also make a meaningful difference to patient care by driving the best possible quality use of medicines. Through your clinical expertise, leadership and collaborative approach, you’ll implement strategies that optimise patient outcomes while ensuring medicines are used safely, effectively and cost-effectively. About You

We’re looking for an experienced and motivated pharmacy professional who brings strong clinical expertise, leadership capability and a genuine commitment to delivering safe, high-quality patient care. You’ll ideally demonstrate:

  • Qualified and registered: Bachelor of Pharmacy (or equivalent), with registration or eligibility for registration with the Pharmacy Board of Australia.
  • Experienced and knowledgeable: Extensive hospital pharmacy experience, with strong technical and professional knowledge of relevant pharmacy standards and codes of practice.
  • A confident leader: Proven leadership, supervisory or management experience, with the ability to educate, train and support professional and non-professional staff.
  • A collaborative problem-solver: Excellent communication, organisation and analytical skills, with demonstrated ability to work autonomously, meet deadlines, undertake research, solve problems and contribute effectively within multidisciplinary teams.
