About the Role

RN2

Join our Transition Care Programme as a Clinical Nurse and help older people move confidently from hospital to the right care, health and rehabilitation services.

  • Coordinate TCP referrals and client intake
  • Develop short term care and case plans
  • Link clients with community and aged care services
  • Support staff and maintain high clinical standards

AHP2

Join our Transition Care Program as a Transition Care Program Coordinator and help older people move safely from hospital to the right community care, health and rehabilitation services.

  • Coordinate TCP referrals and client intake
  • Assess needs and create short term care plans
  • Link clients with community and rehabilitation services
  • Manage ongoing case care after discharge
  • Guide staff and maintain high practice standards
