WHO ARE WE LOOKING FOR?
- Are you a qualified Social Worker with experience delivering person-centred services across diverse client groups and increasingly complex presentations?
- Do you have the ability to work independently while collaborating effectively within a multidisciplinary community health team?
- Can you manage a varied caseload, prioritise competing demands, and apply sound clinical judgement in a busy healthcare environment?
- Are you committed to continuous learning, reflective practice, and supporting the growth of students, peers, or less experienced staff?
- Do you value cultural safety, community engagement, and building strong partnerships with clients, families, carers, and service providers?

HOW WILL YOU CONTRIBUTE?
You will……….
- Deliver high-quality social work services, including assessment, intervention, care planning, advocacy, and evaluation to support positive client outcomes.
- Manage a diverse and increasingly complex caseload while promoting independence, self-management, and wellbeing within the community.
- Contribute social work expertise to multidisciplinary service planning, care coordination, and the ongoing development of community-based programs
- Build strong relationships with consumers, carers, Aboriginal communities, and key stakeholders to ensure culturally safe and responsive service delivery.
- Support quality improvement, service evaluation, professional development, and evidence-based practice across the Community and Allied Health team.
