About the Role

Join CALHN as a Social Worker in the Transition Care Program and help older people achieve meaningful care outcomes. You’ll manage a complex caseload, deliver tailored therapy and work with a multidisciplinary team to support restorative, person-centred care.

• Assess needs and deliver tailored social work therapy
• Manage a complex caseload with sound clinical judgement
• Plan holistic care with clients, carers and service partners
• Build strong links across health, community and aged care
• Support service quality, research and team development

About You

Bring your social work expertise, sound clinical judgement and collaborative approach to supporting older people with complex needs. To be successful in this role you will:

• Hold a degree eligible for full AASW membership
• Hold or be eligible for a South Australian driver’s licence
• Bring experience managing complex older person caseloads
• Deliver social work assessment, therapy and intervention
• Work autonomously and within an interdisciplinary team
