Support at Home Care - Care that keeps people independent at home

As a Support at Home Care Partner Enrolled Nurse, you'll play a vital role in supporting older people to remain safe, independent and well in their own homes.  Working under the direction of a Registered Nurse Clinical Care Partner, you will deliver person-centred nursing care aligned with clinical standards, regulatory requirements and best practice in community aged care.

This role blends clinical nursing with meaningful community connection - supporting clients, their carers, and a multidisciplinary team to achieve the best possible health and well being outcomes.

About You

  • You are an Enrolled Nurse registered with AHPRA and hold a current practising certificate, bringing a minimum of three years' experience in nursing, ideally within aged care or community settings.

  • You are confident in delivering person-centred care, prioritising workloads, and recognising and escalating changes in a client's condition.  Your communication skills are strong, and you work effectively within a team environment while treating older people and their carers with empathy, dignity and respect.

  • You bring a calm, organised and responsible approach to your work, with a genuine commitment to supporting independence and positive health outcomes at home.
