The Virtual Rehabilitation Ward at Flinders Medical Centre provides care to people with complex medical, functional, and psychosocial needs in their own home — in place of an extended or avoidable hospital admission.

About the role: 

Reporting to the Manager, this role is responsible for provide specialist social work assessment, intervention and care planning to people with complex medical, functional and psychosocial needs — supporting them to remain in their own home in lieu of an extended or avoidable hospital admission.

Duties for the role will include: 

  • Commitment to delivering high quality and safe care consistent with the SALHN Integrated Governance Framework and Consumer Engagement Framework and Plan.
  • Independently assessing clients’ abilities, problems and needs using a variety of formal and informal assessments as needed. 
  • Communicating with clients’ family, carers and other service providers.
  • Participating in interdisciplinary clinical and planning activities to enhance the overall service provided to clients.
  • Initiating, developing and contributing to educational activities within the team.

About you:

  • A degree or equivalent in Social Work, which gives eligibility for full membership of the Australian Association of Social Workers
  • Demonstrated competence in the application of a wide range of assessment and therapeutic interventions to clients with complex needs.
  • Ability to critically analyse or evaluate issues related to clinical care in social work in a manner which can be applied to quality activities, service evaluation or research.
  • Experience providing social work services to people in a hospital setting.
  • Proven experience in delivering high quality and safe care consistent with the National Safety and Quality Health Care Service Standards.
