Join the Geriatric Outreach Service (GOS) and become part of a dedicated multidisciplinary team delivering specialist care to residents in Residential Aged Care Homes across Southern Adelaide. This rewarding role offers the opportunity to apply your clinical expertise while supporting older people to remain well, connected and cared for in their place of residence.

GOS is a highly valued multidisciplinary team providing specialist geriatric services across metropolitan Adelaide. As a Clinical Nurse, you will work alongside medical, nursing and allied health colleagues to deliver person-centred care that promotes wellbeing, independence and quality of life.

You will build strong relationships with residents, families, aged care staff and health professionals while providing clinical assessment, care coordination, education and support. Through proactive and collaborative care, you will help prevent avoidable hospital presentations and ensure older people receive the right care in the right place.

This role offers variety, autonomy and the opportunity to work across multiple residential aged care settings as part of a supportive and experienced team committed to excellence in aged care.

About You

We are looking for nurses who are passionate about improving the lives of older people.

You will be an experienced Registered Nurse with at least three years post-registration experience who enjoys working collaboratively and building positive relationships with consumers, families and healthcare partners. You are compassionate, adaptable and confident in delivering high-quality care in a community-based environment.

You will bring:

  • A commitment to person-centred and age-friendly care.
  • Experience in the assessment and management of older people with complex health needs, including Behavioural and Psychological Symptoms of Dementia (BPSD).
  • Strong communication and relationship-building skills.
  • Sound clinical judgement and problem-solving abilities.
  • The ability to work both independently and as part of a multidisciplinary team.
  • A willingness to learn, share knowledge and contribute to service improvement.
  • A genuine passion for supporting older people to achieve the best possible quality of life.
