About the role: 

Reporting to the Advanced Nurse Unit Manager - GEM at Home, this role will be involved in the delivery of goal-focused, restorative physiotherapy to older people in their homes — helping them regain confidence, mobility, and independence after an acute event or functional decline.

Duties for the role will include:

• Assessing patients’ physical abilities, problems and goals using a variety of formal and informal assessments. 
• Participating in interdisciplinary clinical and planning activities to enhance the overall service provided to patients
• Providing information to patients and/or relatives about level of functioning and ongoing management
• Providing a consultative service advice regarding Physiotherapy to other team members, disciplines and agencies
• Contributing to the development of an integrated team approach and culture which is highly responsive to the needs of our consumers.

About you:

• A degree or other qualification which entitles registration as a Physiotherapist with the Physiotherapy Board of Australia.
• Experience as a clinician in the assessment and provision of high-quality Physiotherapy services to clients with a variety of conditions requiring a restorative approach
• Ability to assess and provide appropriate Physio-therapeutic management to the patients of the GEM at Home service with increasing levels of complexity
• Evaluate issues related to Physiotherapy care in a manner which can be applied to quality activities, service evaluation, professional education or research with a commitment to ongoing professional development.
• High levels of responsibility and organisational skills including the ability to prioritise work, meet deadlines, delegate appropriately. This includes the ability to supervise students and allied health assistants.
• High level interpersonal, negotiating, counselling and problem-solving skills with the ability to communicate with clients, other professional staff and community agencies both verbally and in writing.
