About the role

As a VMCH Case Manager, you will play a crucial role in providing comprehensive support to consumers with diverse and complex needs. Your responsibilities will include developing tailored care plans, managing consumer budgets, and ensuring compliance with Aged Care Reforms and Quality Standards, and the HCP legislative framework. Your expertise in Case Management, customer service, and autonomy will be vital in delivering high-quality care and support. Additionally, your willingness to adapt to change and travel as required, along with your commitment to cultural diversity and workplace safety, will contribute to the organisation's mission and values. Your role will involve collaborating with various stakeholders, understanding referral pathways, and upholding the highest standards of care and service delivery.

About you

What you will bring to the role:

  • Health-related tertiary qualification or in a related field.
  • Candidates should be prepared to adapt to changing caseloads and show a flexible mindset.
  • Previous experience as a care manager developing care plans and managing consumer budgets.
  • Understanding of Aged Care Reforms and Quality Standards.
  • Must be willing to travel to regional Victoria when required.
  • Aged Care knowledge.
  • Customer service focus.
  • Willingness to travel.
  • Driver’s license and police check required.
