About the role
The Community Connections Coordinator will be responsible for overseeing our Community Connection sites and programs. Responsible for the day-to-day operations of the sites and provision of care to our consumers. The Community Connections Coordinator will be responsible to support the service growth and quality of service that meet the psychological, cultural, social, and emotional needs of our care recipients.
About you
- Experience in Aged & Community Care, including wellness programs and care plans.
- Certificate III/IV in Aged Care (or similar) and strong communication skills.
- Proficient with MS Office, Client Management software, and Aged Care Reforms.
- Ability to work independently, manage time effectively, and deliver excellent customer service.
- Valid Victorian Full Driver’s License.
- Knowledge of Aged Care standards, experience with diverse populations, and familiarity with Carelink Plus/My Aged Care.
