About the role
As a Case Manager at VMCH, you’ll be responsible for delivering high-quality, person-centred services to clients and carers across designated regions. You’ll ensure all services meet funding body and organisational guidelines, applying consumer-directed care principles from initial enquiry through to ongoing support. This role also involves community engagement and contributes to VMCH’s reputation as a leader in aged care services.
- Conduct comprehensive assessments and develop tailored care plans
- Provide advice on service options and coordinate referrals
- Empower clients and carers to manage their care plans with confidence
- Maintain accurate client records and ensure compliance with funding and legislative requirements
- Monitor service delivery and respond to changing client needs
- Ensure compliance with the Aged Care Quality Standards and relevant legislation
About you
- Tertiary qualification in Health or a related field (nursing or allied health preferred)
- Experience in case management and care planning (12 months minimum)
- Strong understanding of aged care reforms and consumer-directed care
- Excellent communication, time management, and networking skills
- Willingness to travel regionally and hold a current Victorian driver’s licence
- Completion of a National Police Check
